Misplaced your policy! No problem. Visit one of our locations to get a Declaration of Lost Policy for completion.

  • Declaration of Lost Policy (sworn before a Commissioner of Affidavit)
  • Payment for Advertising, Replace of Policy and Stamp Duty (if applicable)
  • Identification for the Insured (Passport, Driver’s License or National Identification)

Note: You will be required to complete additional forms when you visit our branch. 

A withdrawal can be made by accessing the accumulated cash value from your Golden Enhanced Masterplan (GEM) policy once the policy fund value is sufficient. Visit one of our locations to get the cash value on your policy. You will need the following documents to process your withdrawal.

  • Identification for the Insured (Passport, Driver’s License or National Identification)
  • Identification for Irrevocable Beneficiaries (Passport, Driver's License or National Identification)
  • Original Policy Contract

Note: The irrevocable beneficiaries will be required at our office to complete the transaction. You will be required to complete additional forms when you visit our branch. 

Surrendering a policy means the policyholder terminates the policy before the maturity date stated on the policy schedule. When a policy is surrendered there is no obligation to continue premium payments as the policy will no longer be in-forced. The policyholder may have to pay a surrender charge and receive a surrender value depending on the terms and conditions of the policy. If you are interested in this option, you will need to present the following documents at any one of our locations:

  • Identification for the Insured (Passport, Driver’s License or National Identification)
  • Identification for Irrevocable Beneficiaries (Passport, Driver's License or National Identification)
  • Original Policy Contract

Note: The irrevocable beneficiaries will be required at our office to complete the transaction. You will be required to complete additional forms when you visit our branch. 

A policy can only be reinstated if it is lapsed and falls within the reinstatement period as determined by the policy wording. A reinstated policy provides the policyholder with the same coverage and under the same terms and conditions. If you wish to have a policy reinstated, you should proceed to a location nearest to you with the following documents.

  • Identification for the Insured and Beneficiary (Passport, Driver’s License or National Identification)
  • Birth certificate for the Insured and Beneficiary
  • Marriage certificate for the Insured and Beneficiary or affidavits (if applicable)
  • Payment of premium
  • Vaccination card
  • Proof of Address (Bank Statement or Utility Bill must be within 3 months)

Note: You will be required to complete additional forms when you visit our branch. 

The maturity of a policy occurs when the policy reaches the maturity date as stated on the policy contract. When a policy matures the insurance company is obligated to the maturity benefit providing the policy is active. Not all policies will have a maturity benefit some policies only have a death benefit.

  • Original Policy Contract
  • Identification for the Insured (Passport, Driver’s License or National Identification)
  • Birth Certificate
  • Marriage Certificate or affidavit (if applicable)
  • Bank Statement if funds are to be paid via ACH
  • If there is a change to the maturity date proof of BIR number is required 

 

An annuity statement provides the policyholder with the premiums paid for the prior year and a projection for the existing year. This statement is usually requested when the annuitant is filing his/her tax returns. This request can be facilitated either via email or in person. A beneficiary is a person you appoint to receive the death benefit of your insurance policy in the event of your death.

In most instances you can change your beneficiary if they are revocable. If your beneficiary is irrevocable, you will need their written consent to make changes. To add/change a beneficiary you will need to visit our office with the following documents.

  • Original Policy Contract
  • Identification for the Insured (Passport, Driver’s License or National Identification)
  • Identification for the Beneficiary (Passport, Driver's License or National Identification)
  • Birth Certificate for the Beneficiary
  • Marriage Certificate or affidavit (if applicable) for the Beneficiary
